Coverage varies greatly between plans. Please use this Insurance Benefit Resource to help with determining what your plan covers. If insurance declines to cover your visit(s) for any reason, you are responsible for the full amount billed.
I am not contracted with the following:
Aetna (often times nutrition counseling is covered same for out-of-network as in-network)
United Healthcare (can request prior authorization for services)
and all others
If I am out-of-network for your plan, I can in most cases bill your insurance directly when you leave a credit card on file for any remaining balances after insurance processes the claim. Or you can choose to pay the discounted "time-of-service" rate and submit the claim to your insurance yourself.
